What to ExpectAfter scheduling an appointment we will send you a Health Survey Form for you to complete at your convenience at home.Please provide us with all information requested, including Past Medical History, Allergies, Medications (Name, dose and reason for taking). Bring this form with you to your appointment, along with your insurance card(s). Why Is this Information Important?Careful consideration of medical conditions, health history and other factors (for example, activity level, type of work) are necessary in order to develop a treatment plan that is most appropriate for you.At your initial visit, one of our assistants will obtain a brief history of your foot problem, as well as your vital signs. A digital photograph will be taken of your feet, usually while weight bearing, for your file. Gait observation may also be performed. Diagnostic tests may be performed, such as X-rays or Ultrasound. Please anticipate that you will be at our office for a minimum of one hour for your initial appointment. We encourage you to familiarize yourself with our Website so that you may be better informed about Adirondack Tri-State Podiatry and Dr. Donela.
